The following documents outline district procedures for use of Student Activity accounts. Please note that it is YOUR responsibility to be aware of these procedures, and to follow them at all times.

- Purchasing Procedures - must be followed in order to expend funds from your account

Deposit Detail Summary Form - used for preparing deposits. You have three options for downloading this form, two of which will do the calculations for you...

Deposit Detail Attachment/Student List - use this form if you are collecting small amounts from a large number of students for the same deposit. It will enable you to record the amount received by each student and perform calculations to determine the total amount collected by grade level.
NOTE: If using this form, you MUST attach it to the Deposit Detail Summary Form and type in "see attachment" under Name when entering the total amount of cash on the Cash Deposit Detail section of that form.

Account Recording Spreadsheets - in Excel 2007-2010 format, choose the tab that is for your account. You can enter deposits and purchases, it will do the math! You must maintain a spreadsheet for any account you are responsible for, which must be submitted for auditor review in December and June.
The following documents are for correspondence with vendors that have been de-registered from the district database due to non-compliance with recent "Pay-to-Play" reforms:
- Vendor Registration Packet - this needs to be sent to the vendor, who must provide one or more of the following in order to be registered: IRS Form W-9, New Jersey Business Registration Certificate (BRC), New Jersey C.271 Political Contribution Disclosure (PCD) Form.
