Spring Sports Info

Tryouts for SPRING sports, will begin on March 1, 2012.  Physicals completed anytime after March 1, 2011 will be acceptable.
 
ALL forms must be reviewed by the school physician once they are returned, prior to the student being cleared to participate in the sport therefore,  forms MUST be in the BMS health office NO later than
February 27, 2012 to be eligible for participation.

Physical forms are available in the BMS health office or may be downloaded HERE.
 
REMINDER:  Forms may be dropped off or mailed to BMS during the summer break to avoid any delay in fall sport clearance.
 

SPORTS PHYSICALS PROCEDURE:

According to New Jersey State Mandates, each candidate who wishes to participate in athletics, school athletic squad or a team MUST be examined by their primary health care provider within 365 days PRIOR to the first tryout session.

The New Jersey State Annual Athletic Pre-Participation Physical Form [.pdf file]MUST be filled out by the parent and primary health care provider and submitted to the nurse at least 2 WEEKS prior to tryouts for review and clearance from the school physician.
