Kindergarten Registration will be held:

Required Documents

1. Lease or deed from your residence *
2. Copy of Cable or Electric Bill
3. Immunization Records
4. Original Birth Certificate

*If you are not the homeowner and you live with a relative who is the homeowner you must bring an approved Affadavit from the Superintendent (call 973-263-7200 x7250 to schedule an appointment).

Children entering kindergarten must provide evidence of immunizations. Students who have not met all immunization requirements will not be permitted to attend the first day of school.

IMMUNIZATION REQUIREMENTS
DPT	a minimum of 4 doses provided at least one dose is given on or after the 4th birthday.
Polio	a minimum of 3 doses provided at least one dose is given on or after the 4th birthday.
Measels	2 doses given after the first birthday (preferably MMR).
Mumps	at least one dose (preferably MMR) given on or after the 1st birthday.
Rubella	at least one dose (preferably MMR) given on or after the 1st birthday.
Hepatitis B	3 doses prior to entering kindergarten.
Varicella	1 dose after first birthday.
Mantoux	Intradermal test using 5 TU of stabilized PPD tuberculin.
Pneumococcal Conjugate *New*	Children who are 2 to 59 months of age must receive this vaccine.
Influenza Vaccine *New*	Children who are 6 to 59 months of age must recieve one annual dose by December 31 of each year

Parents must have their family physician do a physical examination prior to school attendance in September.

For the 2012/2013 school year Lake Parsippany School has the capacity to register 50 kindergarten students. A kindergarten registrant who has a sibling currently enrolled in Lake Parsippany School is guaranteed a spot in kindergarten at Lake Parsippany. All other registration is on a first come - first served basis. Should more than 50 students register for kindergarten, the extra students will be assigned to open spaces in another elementary school.

We are anticipating that many parents will feel the need to be present on the Monday night of registration. If your kindergarten age child is a sibling of a Lake Parsippany student, consider registering on Tuesday morning to avoid the crowd.
