Spring Sports:
All athletes must have a completed physical on file in the
health office dated AFTER
03/02/2011
to be eligible for the Winter season.

Please have your paperwork back to the
Health Office no later than Monday 02/06/2012

These papers MUST be signed
by the students and parents, and returned to the
school nurse along with the medical information.

Any participant who is medically excused by a nurse, doctor or trainer, cannot participate in school team practices or games until cleared by a doctor or school Athletic Trainer. Furthermore, any student who is medically excused from participation in Physical Education cannot participate that day in the school team practice or game.
