Fall sports info:

Tryouts for fall sports will begin on September 11, 2012, physicals completed anytime after September 11, 2011, will be accepted.
Forms MUST be in the health office NO later than September 10, 2012 to be eligible for participation.

Physical forms are available in the BMS health office or may be downloaded HERE.
 
REMINDER:  Forms may be dropped off or mailed to BMS during the summer break to avoid any delay in fall sport clearance.
 
SPORTS PHYSICALS PROCEDURE:

According to New Jersey State Mandates, each candidate who wishes to participate in athletics, school athletic squad or a team MUST be examined by their primary health care provider within 365 days PRIOR to the first tryout session.

The New Jersey State Annual Athletic Pre-Participation Physical Form [.pdf file]MUST be filled out by the parent and primary health care provider and submitted to the nurse at least 2 WEEKS prior to tryouts for review and clearance from the school physician.
